As Jason undergoes surgery, Elizabeth, Monica, Carly and Sam envision alternate realities with Jason.
In Jason's hospital room, he debates the pros and cons of having surgery with Sam. She says it's his decision, but he wants them to decide together.
At the hub, Robin tells Patrick, should Jason agree to surgery, she wants Matt to assist. Patrick says, "No way." Across the way, Matt and Liz muse on her culpability in Siobhan's death. She doesn't think Lucky will forgive her. Matt suggests she forgive herself first. Matt joins Robin and Patrick's argument, which turns into a fight between the brothers. Robin reminds them Jason's life is on the line. She wants real arguments regarding Matt assisting. Patrick says Matt's focus will be on him, not Jason, so he shouldn't be in the O.R. Elizabeth agrees. As the debate rages on, Carly and Michael approach. Sam walks up and tells the group Jason's decided to have the surgery. Patrick says they will do it tonight. Edward and Monica walk up, as Matt disagrees with Patrick in front of Sam. Sam lashes out at him and he backs off. Matt will take himself out of the equation. He gets on the elevator, as Liz tries to explain why she sided with Patrick. Monica tells Sam she made the right choice. Later, Monica talks with Edward about how Jason changed so completely after his first accident. She wonders if it would be awful to want him to change back again. Edward smiles and says, "Be careful what you wish for."
Carly visits Jason in his room. He tells her he and Sam decided together to have the surgery. Carly is clearly upset by that, but says she just wants him to be okay. She's got a lot of stuff going on and can't handle it alone. She tells him Michael wants to take his place with Sonny while he's incapacitated. Jason orders her to get Michael right now. Sam walks in and yells at Carly for dumping her crap on him. Sam wants Jason to put himself first just once. Jason and Carly express in their own ways how much they mean to each other and she leaves. As Sam lies with Jason on his bed a nurse stops by to tell them it's time.
After Jason is wheeled into surgery, Sam heads to Jason's room and lies in his bed. She fantasizes about them being on their tropical honeymoon, which they've been on for six months. Sam has gone back to salvage diving and has found an item that could lead them to a bigger treasure. She enlists Jason's help and as they look at a map, a man approaches with a gun.
Liz goes to her art studio. She imagines walking in and finding Jason, who is an architect, sleeping on his table. She wakes him up and they kiss. He tells her he got a job in Tokyo for six months. She's not happy he'll be gone for that long, but Jason thinks she should understand since she's an artist too. They start making out and then Jason finds her birth control pills, which have not been opened. He asks if she's trying to get pregnant.
Carly walks into Jake's and imagines them meeting up there to play pool as a couple. In her fantasy, Sonny had a complete breakdown, burned many bridges and left town. Jason is in charge and Carly has left the hotel indefinitely to help him. She marvels that they are partners in the truest sense of the word. Michael enters and angrily demands to know what his mother is doing with that "gangster garbage."
Monica returns home and envisions Alan entering the room to tell them Jason, who never got in the car with A.J., has been named chief of staff. Tracy and Edward join the celebration, with Tracy being her usual disdainful self. Alone, in the Q living room, Jason does a line of coke, as fantasy Monica walks in on him.
Robin watches from the observation area as Patrick begins surgery. She hopes she made the right call letting Patrick operate alone. Patrick hits a few snags during the operation.
