Olivia and cousin Connie grew up like sisters in Bensonhurst, New York. In high school, Olivia dated Sonny Corinthos, but eventually lost him to his true love, Connie. After High School, Connie changed her identity and moved to Manhattan to make a new start. Olivia and the rest of the Falconeri family were hurt and angry.
Years later, Sonny reached out to Olivia and invited her to his wedding to Connie, who was now going by the name Kate Howard. Olivia came to Port Charles and helped her cousin get ready for her wedding. They had their first of many fights on Kate's wedding day about her denying who she really was by changing her name. Connie never married Sonny due to her being shot. Olivia stayed by her side until she recuperated and got a job as the manager of the Metro Court.
Olivia and Kate remained on strained terms, but Olivia also found herself drawn to Sonny. Nothing ever came from their renewed flirtation, as Olivia began a steamy affair with the much younger Johnny Zacchara and she was keeping a huge secret: She got pregnant by Sonny when she 16 years old and never told him about it. She kept her baby, who turned out to be undercover cop Dante Falconeri, who was posing as Dominic Perelli, who was infiltrating Sonny's organization.
Only Kate knew about Dante (and Patrick Drake after Kate let it slip) until Sonny shot Dante after finding out he was cop. Olivia found her son in a pool of blood and screamed at Sonny that he just shot his own son. Dante recuperated of course and Sonny tried to form a relationship with him. Olivia largely stayed out of it and focused on her relationship with Johnny.
Considering their age difference and Johnny's line of work (mobster), the two eventually broke up. Olivia began a flirtation with Dr. Steve Webber, who tended to her when she became seriously injured on the hospital sponsored ski trip they were chaperoning. The bus crashed leaving Olivia with internal injuries. Once brought to the hospital, Patrick operated on her and she survived.
Olivia and Steve began a full fledged relationship, but it became apparent he was hiding something. The arrival of Steve's ex from Memphis, Maggie, only deepened the mystery. It came out Steve pulled the plug on a felon who was in his care so one of Maggie's patients could receive his organs. Olivia stood by her man and tried to get Johnny, who was blackmailing Steve, to back off.
During the same time, Olivia had to deal with Steve's crazy mother Heather who had returned and her unraveling cousin Kate, who developed a split personality Connie.

At the station, Franco warns Scotty to tread carefully and laughs that he wouldn't tell Lucy about that nightcap he had with Bobbie. Scott doesn't know what he's talking about. Franco tells him about arriving home last night and Carly explaining away the wineglasses and saying Scott had called Bobbie for a nightcap. Scott says he met her for dinner. Franco thinks something's fishy. Scott warns him not to get in the middle of Carly and Sonny - he's a bad guy. He has a sinking feeling the story's not true. Nearby, Sonny assures Olivia that nothing will happen to Lulu with Dante on the case. Carly arrives, upset about Lulu, and Sonny caresses her shoulder. They end up bickering about Franco. Carly turns and Franco has disappeared. Sonny tells her when she realizes she's made a mistake, he'll be there. Carly finds Franco with Scott as Franco is demanding to know what time Scott called Bobbie for dinner. Franco covers and notes he saw Carly with Sonny. Carly says it was about Lulu. Scott leaves, and tells Franco they're done - he's grasping at straws. Meanwhile, Dante arrives and comforts Olivia. Sonny tells him Anna's not there. Dante freaks out a bit. Olivia talks him down and they leave together. After, Franco and Carly run into Sonny on their way out. Franco warns Sonny to stay away from his girlfriend.
At home, Dante checks in with his child while Olivia looks on.
At The Floating Rib, Olivia and Ned discuss Coleman’s arrest. Tracy arrives and starts slinging insults at Ned and his “Bensonhurst Bimbo.” Ned interrupts and tells her about the kidnapping. Olivia leaves to call Sonny and Tracy downs a drink. She loves Lulu like a daughter but can’t get in touch with Luke when she calls. Carly and Franco enter and Ned gets them up to speed on the kidnapping. Carly wants to call Sonny but Franco encourages her to let Olivia take care of Sonny. Tracy has another drink and leaves another message for Luke. Ned guesses he has more important issues than his daughter.
Lucy goes to PCPD, demanding to see Anna about Maxie and Lulu. Scott arrives, glad to see her and her legs. Lucy asks him for an update and gets upset thinking of Serena. He reminds her how strong and smart she is, and Lulu and Maxie are just as strong. Scott assures her they’ll all be okay. Lucy then demands Scott make a decision about her or Bobbie. He hasn’t made up his mind and she’s devastated that he doesn’t know what he wants. Carly comes in like gangbusters and she and Franco asks for any info Scott has. He reveals Scribner was shot and they need him to wake up to find out where the girls are. Later, Lucy puts Scott on notice. If he doesn’t make up his mind soon he could lose them both. She leaves and Franco makes a comment to Scott about his nightcap the other night with Bobbie. Scott has no idea what he’s talking about. Meanwhile, Sonny and Olivia show up. He assures Carly and Olivia that Dante will do what it takes to bring Lulu home.
