Olivia and cousin Connie grew up like sisters in Bensonhurst, New York. In high school, Olivia dated Sonny Corinthos, but eventually lost him to his true love, Connie. After High School, Connie changed her identity and moved to Manhattan to make a new start. Olivia and the rest of the Falconeri family were hurt and angry.
Years later, Sonny reached out to Olivia and invited her to his wedding to Connie, who was now going by the name Kate Howard. Olivia came to Port Charles and helped her cousin get ready for her wedding. They had their first of many fights on Kate's wedding day about her denying who she really was by changing her name. Connie never married Sonny due to her being shot. Olivia stayed by her side until she recuperated and got a job as the manager of the Metro Court.
Olivia and Kate remained on strained terms, but Olivia also found herself drawn to Sonny. Nothing ever came from their renewed flirtation, as Olivia began a steamy affair with the much younger Johnny Zacchara and she was keeping a huge secret: She got pregnant by Sonny when she 16 years old and never told him about it. She kept her baby, who turned out to be undercover cop Dante Falconeri, who was posing as Dominic Perelli, who was infiltrating Sonny's organization.
Only Kate knew about Dante (and Patrick Drake after Kate let it slip) until Sonny shot Dante after finding out he was cop. Olivia found her son in a pool of blood and screamed at Sonny that he just shot his own son. Dante recuperated of course and Sonny tried to form a relationship with him. Olivia largely stayed out of it and focused on her relationship with Johnny.
Considering their age difference and Johnny's line of work (mobster), the two eventually broke up. Olivia began a flirtation with Dr. Steve Webber, who tended to her when she became seriously injured on the hospital sponsored ski trip they were chaperoning. The bus crashed leaving Olivia with internal injuries. Once brought to the hospital, Patrick operated on her and she survived.
Olivia and Steve began a full fledged relationship, but it became apparent he was hiding something. The arrival of Steve's ex from Memphis, Maggie, only deepened the mystery. It came out Steve pulled the plug on a felon who was in his care so one of Maggie's patients could receive his organs. Olivia stood by her man and tried to get Johnny, who was blackmailing Steve, to back off.
During the same time, Olivia had to deal with Steve's crazy mother Heather who had returned and her unraveling cousin Kate, who developed a split personality Connie.

At trial, Diane objects to allowing security footage from the Metro Court to be entered into evidence. The defense was never given a copy. Sonny yells that the proof should be allowed. The judge shuts him down only to have Scott agree that the jury should see that AJ is a cold-blooded murderer. The judge allows the court to see the video without the jury. Kiki asks Michael if he's sure he wants to stay. He has to – for both of his fathers. Olivia suggests that she and Sonny leave. He won't. Diane warns AJ to control his looks when the jury returns. Scott starts the video and AJ is seen falling over drunk, making his way to the elevator. Diane's objections are overruled and the video continues to show AJ pull a gun out of his pocket on the way to Crimson. AJ thinks he really did it and says, "I killed Connie." Diane doesn't have a problem with the footage as long as they see the rest. Scott doesn't have more footage, it was destroyed. The judge rules the footage won't be allowed but AJ thinks he killed Connie. Diane is certain that no one will know what really happened until the remaining footage is found.
Dante asks Lulu if she's okay as she carries Connie around the apartment making plans to take her to the park. Olivia and Sonny arrive to say their goodbyes. Without baby Connie, they wouldn't have gotten through losing Connie. Lulu can't believe Maxie will be raising her. Olivia announces she won't – Spinelli won custody. They leave and Lulu pines over their hollow victory and blames Dante for their loss. He lied about Sonny shooting him, why couldn't he lie to save his wife or daughter? Dante couldn't because Connie isn't their daughter. She didn't belong and they have to face this. Dante confesses his love as they bond with Connie until Spinelli arrives. They say their goodbyes and hand Connie to Spinelli. Lulu collapses in Dante's arms at the door.
Kiki stops Michael at the court house to pick up a heads-up penny for him. Monica joins them as AJ is brought out in shackles. AJ's grateful for the support but one person is missing, Elizabeth. Outside the court room, Sonny confronts Scott with Olivia on his arm. He believes he's in Julian's pocket and doesn't want him to lose AJ's trial. Scott warns him for threatening him. Diane points out that Scott has a lot to lose when she shows up. They follow Elizabeth in who doesn't have a chance to talk to AJ. Opening arguments begin with Scott certain that the jury will find AJ a cold-blooded killer. Diane claims there is a miscarriage of justice and the real murderer is still out there. Sonny's first on the stand. He recounts the night AJ choked and threatened Connie before explaining how he found her the night she died. He confirms he found 'AJ' drawn in Connie's blood. On cross examination, Diane suggests Sonny had motive to write 'AJ' himself but Sonny denies it. Scott calls Elizabeth next who didn't see AJ strangling Carly at Sonny's but did hear him threaten her there and later, when he told her "if he had a gun he'd kill her." Olivia yells at Elizabeth for not saying anything and AJ hangs his head down as Elizabeth leaves. Olivia's called to the stand to authenticate security footage from the Metro Court of the night Connie was murdered.
