Helena Cassadine entered the story as a villainess and quickly became the resident 'evil' person, visiting sporadically to put everyone on edge at the mention of her name. Her first act of unkindness was the spell she placed on Luke and Laura for foiling her plot to freeze the world. She spoke the damning words and then disappeared.
Her main concern has always been the Cassadine fortune and her need to control it. Her grandson, Nikolas is the heir and the object of her affection. Helena has gone to great lengths to maintain control over him and the money. She has never liked Emily, and has taken great strides to remove her from her grandson's life. Unfortunately, one of Helena's attempts almost ended in her own demise, when Nikolas pushed her off a cliff. The woman is seemingly invincible and continues to defy death. Her nemesis is Luke Spencer, and the two have locked horns on many occasions.
Helena never relents, using whatever method necessary to get her way. Murdering Alexis' mother, kidnapping Michael Corinthos, plotting to get rid of Nikolas' parents, Stefan and Laura, brainwashing Lucky, and finally hiring an unscrupulous nanny for Nikolas' child are just a few of her demented acts. Even insane asylums can't hold her. She lurks in the shadows and always knows everyone's dirty secrets, then uses them to her own advantage.
Despite the belief that she was safely tucked away in yet another mental facility, Helena made an appearance at Luke and Laura's remarriage in 1996. One of Laura's last moments before returning to her catatonic state was spent face-to-face with the evil Helena Cassadine as she hexed their joining, once again. Who knows what the vengeful woman will do next.

At Liz’s place, Jake/Jason answers the door to find Helena Cassadine standing there. She claims to be an old friend of the family, so he invites her in. Jake thinks she looks familiar and he wonders if they met before, explaining he’s lost his memory. She claims she just has one of those faces. Jake decides to go get Elizabeth, but when he leaves the room, Helena slips out. He unfortunately didn’t get her name to tell Elizabeth. Elizabeth thinks perhaps it was just a memory of his resurfacing. Jake knows the woman was real and heads to the door, thinking she couldn’t have gone far. When he opens it, Sam is standing there. Sam, Patrick and the kids head inside and Danny once again takes to Jake.
Back at Wyndemere, Nathan arrives for dinner with his sister and Obrecht. Outside Helena appears and stops Faison from going inside, pointing out that Nathan is the police. She reminds him of the job he has to do. Faison says he doesn’t take orders from her, and she is supposed to be dealing with Jason Morgan. Helena warns him not to cause their plans to go up in smoke. Back in the mansion, everyone gets ready to sit down for dinner. Spencer realizes forgot his pilgrim hat, so Obrecht goes back to the living room to get it for him. Seeing Obrecht alone, Cesar takes the chance and reveals himself to her.
Back at Elizabeth’s, Helena returns and spies on the gang from the window as they eat dinner.
Luke groggily wakes up in a padded room at Miscavige and finds Helena looming over him. He assumes she's a hallucination and recalls shooting her. "It was not a silver bullet," she says. She cuddles him and purrs, "Love live the queen." When she tries to kiss him, he leaps away. She taunts him. He demands to know who has been living his life. Helena puts a blade to his chin and explains that there cannot be two Lukes. Meanwhile, Julian and Alexis arrive. Julian explains that Luke has been there almost a year. As they head for Luke's room, Helena walks out. "Evil doesn't die," Alexis sneers. Helena would like to slit her throat, just like she slit her mother's. She wanders off. Alexis shakes her head. Julian wonders who that was. The lawyer walks into Luke's room and gasps.
Jake has a nightmare on Liz's couch. He twitches violently until she wakes him. He grabs her wrist. She wonders what he was dreaming of. Jake doesn't know. She asks if he can carve a turkey. Later, he has a shower and then wanders around looking for his shirt. Liz gives him some news clothes to match his eyes. He can't accept the gift. She thinks he should just pretend it's his birthday. After he gets dressed, Patrick calls to ask if he can bring Sam. She agrees. Jake sets the table while Liz gets dinner ready. He answers a knock at the door. It's Helena.
