Monica was raised as an orphan, and as an adult, the closest person to a mother she had was Gail Adamson. It was Gail who encouraged Monica to become a doctor. While in medical school she met her first love, David, who later went to Viet Nam and was reported MIA. Monica assumed he was dead.
Soon after, she discovered she was pregnant and decided to put the child up for adoption so she could finish school. Rick Webber, another handsome med. student soon caught her eye, and they fell in love. After receiving their degrees, Rick went off to Africa, was reported killed in a plane crash, and once again Monica was devastated. She found solace in the arms of Rick's brother, Jeff.
Their marriage floundered from the beginning. Jeff was possessive and insecure of Monica's lingering feelings for Rick, and when Rick, amazingly, turned up alive, Monica wanted to divorce Jeff, but needed his permission. She received unsolicited help from Heather Grant, a woman who had her eye on Jeff and wanted him for herself.
Smooth waters didn't flow as Monica had planned. Dr. Leslie Faulkner was in love with Rick, too, and when Rick found brother, Jeff, with a bullet wound to his head, Rick was overcome by guilt and blamed himself because of his relationship with Monica. He broke it off with her.
Jeff recovered, and he and Monica gave their marriage another chance. Rick and Leslie wed, and Monica realized she didn't love Jeff and asked for a divorce. When he finally agreed, she was free to pursue a new doctor at GH, Alan Quartermaine. They eventually exchanged vows, but with Rick still at GH, Monica gave in to her old feelings and slept with him. When she became pregnant, she didn't know who fathered the child.
Alan's greedy sister, Tracy was determined to prove that Monica's baby was not a Quartermaine. She wanted her own son to be the family heir. Luckily, Monica's child, AJ, was later deemed to be Alan's son.
The stress of Monica's cheating led Alan to have his own affair with Susan Moore, which produced a son, Jason. After a time, Alan realized that Monica was really the love of his life, and he returned home with the baby. At first the little boy was a constant reminder of her husband's infidelity, but Monica soon grew to love Jason as much as she did AJ.
The Quartermaine's marriage was a continuing string of affairs and distrust. Old flames, previously assumed dead, showed up alive, and Skye Chandler, another child from one of Alan's flings, came to Port Charles to find her father. Monica battled breast cancer, befriended another victim of the disease, and when the woman died, Monica and Alan adopted little Emily. Life was not easy despite being wealthy.
Sadly, Monica lost many members of her family over the next few years including, AJ, Alan and Emily. While grieving Emily's death, she turned to the bottle and hit Sam while driving. Jason helped her though and she went to rehab. Monica also took to newcomer Rebecca, who looked just like Emily. However, it came out that Rebecca was Emily's unknown twin who came to town to fleece the Quartermaine's and Nikolas. Since then, Monica has only sporadically appeared at the hospital or in the Q mansion.

At the hospital, Dr. Obrecht corners Franco and tells him how great he's been for the pediatrics ward. He's happy to be contributing to society. He goes back to work. Meanwhile, Tracy is anxious for news about Rafe's heart. She tells Monica about it. Everyone tries to keep her calm. Down the hall, Kiki and Silas discuss the transplant. Rafe's blood-type is a match for Alice. She apologizes for the timing of this. He doesn't see what difference the timing would make and agrees to talk to Alice's people. When they find them, Silas says he's willing to donate his nephew's heart. Franco strolls into the middle of the conversation. They tell him to get lost and he comments on how rude the Quartermaines are. After he leaves, Silas says that Rafe had a good heart, in spite of any mistakes he made. Monica is optimistic that things will work out. Morgan is thrilled and gives Kiki a hug. Michael stares at them. When Monica and Silas start comparing Rafe and Alice's heart in his office, Obrecht interrupts and wonders what they are up to. They fill her in. "No death wasted," Obrecht declares as her motto. The doctors return to the family and tell them that Obrecht has signed off on the transplant. A moment later, Obrecht interrupts to declare there is a problem. She explains that Rafe tested positive to heroin and the hospital can't allow a transplant in that situation. In Rafe's hospital room, Nina thanks Rafe for helping her drive a wedge between Sam and Silas. His hand moves and he stares at her. She keeps herself calm, but Rafe revives and yanks the oxygen tube out of his mouth. He taunts her and threatens to expose her secrets. She yells threats until Franco interrupts. She snaps out of it. Rafe is still comatose. Franco tells her that he used to see and hear things too. He hopes she won't be destroying lives like he did but he assumes she's just under a lot of stress. He leaves as Silas arrives.
Alice wakes up at the hospital. Monica reminds her that she had a heart attack. When Alice tries to say something to Michael, Tracy stops her. Monica explains that Alice's heart is failing and she needs a transplant. She's at the top of the list. After everyone else leaves, Alice tells Michael they need to talk. Down the hall, Morgan worries about Alice to Kiki. He's never seen her sit down let alone stuck in bed. The topic turns to their relationship and how much they lied to each other. They think they've moved past all of that and are friends now. They hug and share an awkward moment. She runs away. By the nurse's station, Nina wheels around and listens in as Patrick tells Silas and Sam that Rafe is brain dead. Silas accuses Patrick of making this happen on purpose. Sam gets between them and drags Silas away to see Rafe. Sitting at Rafe's bedside, Silas beats himself up for failing him and then cries. Kiki arrives. Sam thanks Rafe for coming into her life and promises Danny will never forget him. When Silas is left alone, he tells Rafe that he taught him to open up his heart again and he loves him. He promises to make Patrick pay if he was responsible. Nina shows up and puts her arm on his shoulder. Kiki goes back down the hall to cry on Morgan's shoulder. Meanwhile, Patrick goes down the hall for a shower and thinks about the operation. He gets dressed in the locker room as Felix arrives. Patrick fills him in on what's happened. He's not sure what to tell Sabrina. Later, as Felix takes a shower, Brad interrupts and tells him he has a bone to pick with him. And Sam corners Patrick to ask him if he deliberately botched Rafe's operation.
