Nina Reeves Clay first arrived through recollections of her husband, Dr. Silas Clay. He loved Nina, who had put him through medical school, but he cheated on her with Ava Jerome. Shortly after, Nina was injected with an overdose of antidepressants and went into a coma. The source of the injection is still unknown.
Her mother Madeline Reeves arrived in Port Charles shortly after Nina's brother James Reeves was hired at PCPD to investigate Silas for Nina's attempted murder. It was later revealed that James was not Nina's brother, but rather her cousin.
At first Madeline told everyone that Nina was dead and was arrested for her murder.
On May 1, 2014, Nina woke up from her coma in an undisclosed location and called out for Silas.

Obrecht leads the press conference at General Hospital. The reporters pepper Patrick with detailed questions about his statements intending harm to the patient. He looks at Sam, who is staring at Silas. Nina looks happy off in the corner. Obrecht demands the source but the reporter won’t divulge. Obrecht declares the press conference over and threatens to call security. Patrick pulls Sam aside and she promises she didn’t say anything. Patrick asks Silas if he told the reporters and Sam wants an explanation. Did he tell his wife? Nina promises Silas didn’t tell her anything and Silas accuses Sam of believing Patrick over him. Sam thinks what Silas did is unforgiveable. He swears on his life he didn’t tell a reporter anything and takes Nina home. Patrick apologizes to Sam but she’s the one who is sorry. She thought she could trust Silas. Obrecht returns and tells them the lawyers have only one course of action. She fires Patrick.
At the hospital, Patrick prepares for the press conference. Sam thinks she convinced Silas that Patrick did everything he could to save Rafe, and Patrick thanks her for believing in him. Meanwhile, Anna accuses Obrecht of setting Patrick up for a fall, and that she should let both sides calm down before adding fuel to the fire. Obrecht informs Anna that Rafe had been doing coke and heroin and suggests she stop the flow of illegal drugs into the city and leave the hospital to her. Meanwhile, Nina tells Rosalie that she heard Sam tell Silas in confidence that Patrick thought about killing Rafe, and she wants that tidbit to slip out during the press conference. Silas arrives and brushes Nina off to see Sam, so Nina directs him over to where she is chatting with Patrick. He fumes watching them and Nina offers to take him home to take care of him. Instead, he just walks off as Nina continues to beg after him. The reporters arrive and Nina pretends to drop her bag and after getting some help, offers some help of her own. When the press conference begins Obrecht goes over the details of Rafe’s critical car accident. One of the reporters calls Drake’s surgery a conflict of interest, but Obrecht is sure if they had not operated, Rafe would have died. The reporter needles about the possibility Drake killed Rafe. Patrick assures him he did everything to save Rafe’s life. The reporter asks him why he admitted he wanted the boy dead and the crowd gasps and flashes go off. Patrick looks right at Sam.
