Lucas was born to Cheryl Stansbury. She was told he was stillborn when he was really sold to Bobbie Spencer after an adoption she was planning fell apart and she was desperate for a baby.
Cheryl began to see herself in baby Lucas, as well as Robert Scorpio, the man she thought was the father of her dead child. Bobbie began to investigate Lucas’ background after he was diagnosed with juvenile diabetes, and learned he really was Cheryl’s son. At the same time, Cheryl began to doubt her child really died at birth and hired Robert to dig up the truth. Cheryl led Robert to believe mobster Julian Jerome was the baby’s father. Of course, she thought this was a lie when it was really the truth.
Robert found out that Lucas was Cheryl's baby and was able to prevent Bobbie from skipping town with Lucas. Robert gave Lucas back to Cheryl. She told Robert he was the baby’s father, but a DNA test said otherwise.
After Cheryl died in a car accident, her sister Tiffany Hill and her husband Sean Donnelly returned to Port Charles to care for Lucas, but Cheryl left the baby to a shocked but elated Bobbie and Tony in her will. Tiffany did everything to gain custody of Lucas, even disclosing Bobbie's prostitute past during a custody hearing. Sean refused to testify because of his disgust with Tiffany’s tactics. Bobbie and Tony were left to raise Lucas, but they eventually divorced in 1996.
In 2001, Lucas was poisoned by Lucky Spencer at Helena Cassadine’s bidding in order to control Tony. Tony agreed to do whatever Helena asked in exchange for a cure for Lucas. Tony discovered that Helena was keeping her son Stavros cryogenically frozen in a secret lab underneath the hospital. Tony turned to Luke Spencer for help, and he was able to get Lucas’ cure.
After a few dating mishaps with his adoptive cousins Maxie and Georgie Jones, Lucas came out in 2006. Tony readily accepted Lucas, but died a few months later, which devastated Lucas. He moved to Seattle shortly after, and Bobbie followed.
Bobbie returned to Port Charles when her daughter Carly Jacks went missing. Back in town she learned that Lucas’ mobster bio-dad Julian was still alive. She tried to hide it from Lucas, but Julian had learned about Lucas and was intent on meeting his son. Lucas came back to Port Charles to give Bobbie support and was shocked to learn about Julian. He went out to The Floating Rib for a drink where he met Brad Cooper. The two commiserated over being adopted out of mobster families. The two had a one-night stand. Lucas wanted more, but Brad told him he was interested in another man, Felix DuBois.
When Felix broke a date with Brad, Brad went out for a drink with Lucas instead and they ended up back in bed.
Lucas went to visit Tony Jones’ grave on the anniversary of his death and ran into Julian. Their relationship was already strained, and when Lucas told Julian he was gay, he did not react well. Julian assumed if he had been in Lucas' life Lucas might be straight.

Alexis is with Julian at his penthouse. He talks about how some things don't work out. She promises to stay by his side as he leaves the mob. Lucas shows up with balloons. "Congratulations!" he bellows. He babbles until the lawyer stops him. They fill him in about what happened with the baby. He takes the balloons away.
Sam, Kiki, Morgan, Lucas and Alexis gather around Julian at the penthouse. They refuse to leave him alone. He thanks them. This is what he needed today. He announces that he's giving up organized crime. Suddenly, what appears to be a disguised Ava arrives.
At the ball, Brad proposes to Lucas in front of everyone, and Lucas accepts. Julian welcomes Brad to the family. Lucy runs over to congratulate them as well, but also to let Lucas know the Magic Milo act is up next. The group performs a striptease to INXS’ "New Sensation." Meanwhile, outside the ballroom Jake is upset at Carly for what she did to Liz and wishes she could have done things a little differently. However he thanks her for exposing Ric and Hayden. He realizes he can be with Liz now, but Carly thinks he still may have a life and family out there. Jake says he’ll deal with the fallout if and when he remembers, but right now he wants to be happy with Liz. He says it would be nice if she had his back, and she tells him of course he does. In a dressing room, Nikolas tells Liz that Jake is actually Jason. She believes he’s lying, so he tells her the entire story about Helena holding him at Crichton Clark, and Jason being hit by a car the night of the explosion. Liz realizes this is why she and Jake had such an instant connection, and why Jason chose the name Jake. She asks when he found out, and when he tells her she is furious that he learned months ago. She demands an explanation, so he tells her all about his scheme to takeover ELQ and how Jason’s return would have ruined it. Liz slaps Nikolas. She can’t believe he has done this as Jason was nothing but nice to him. Nikolas knows this, but says he is reclaiming his life after being jerked around these past two years, and he thinks Jason and the people who loved him are all better off this way. He points out that Jason was always torn between his family and the mob, and Sam has finally moved on with her life. Liz plans to tell Jason the truth, and he says he won’t stop her. Backstage Emma comforts Cameron, who feels bad for his mother. Spencer arrives dressed in a cape and looking like the Phantom of the Opera. He stops them from going to the stage to perform and wants Emma to be with him. He feels she’s not because of his face. She tells Spencer that his face has nothing to do with it, and it has to do with the way he treats her like a prize. Emma pushes past Spencer in order to get to the stage. The Haunted Starlets perform the next number and dance to Meghan Trainor’s "Dear Future Husband." Afterwards Cam and Emma dance the tango. During their performance Spencer drops sandbags from the catwalk above the stage, causing the audience to gasp. Spencer’s image appears on the big screen proclaiming to be the Phantom of the Nurses’ Ball and that he has decided to be the monster they all believe he is. After some smoke bombs he appears on stage and tells them all to beware the mysterious pox he is about to unleash upon the ball. Lulu grabs Spencer and drags him off to find his father. Backstage, Lucy tells Duke that it’s clear he still loves Anna, so he should go fight for her. Duke thinks there is no point. As they argue, Lucy changes her outfit, only to have the curtains drawn on them. Everyone gasps at Lucy in her underwear. Sloane asks Anna if this is normal, and she says yes. Lucy uses the situation to introduce Duke as their next performer and says he’ll be dancing the tango with his special guest Anna. Lucy runs off and leaves Duke and Anna to tango.
