Lucas was born to Cheryl Stansbury. She was told he was stillborn when he was really sold to Bobbie Spencer after an adoption she was planning fell apart and she was desperate for a baby.
Cheryl began to see herself in baby Lucas, as well as Robert Scorpio, the man she thought was the father of her dead child. Bobbie began to investigate Lucas’ background after he was diagnosed with juvenile diabetes, and learned he really was Cheryl’s son. At the same time, Cheryl began to doubt her child really died at birth and hired Robert to dig up the truth. Cheryl led Robert to believe mobster Julian Jerome was the baby’s father. Of course, she thought this was a lie when it was really the truth.
Robert found out that Lucas was Cheryl's baby and was able to prevent Bobbie from skipping town with Lucas. Robert gave Lucas back to Cheryl. She told Robert he was the baby’s father, but a DNA test said otherwise.
After Cheryl died in a car accident, her sister Tiffany Hill and her husband Sean Donnelly returned to Port Charles to care for Lucas, but Cheryl left the baby to a shocked but elated Bobbie and Tony in her will. Tiffany did everything to gain custody of Lucas, even disclosing Bobbie's prostitute past during a custody hearing. Sean refused to testify because of his disgust with Tiffany’s tactics. Bobbie and Tony were left to raise Lucas, but they eventually divorced in 1996.
In 2001, Lucas was poisoned by Lucky Spencer at Helena Cassadine’s bidding in order to control Tony. Tony agreed to do whatever Helena asked in exchange for a cure for Lucas. Tony discovered that Helena was keeping her son Stavros cryogenically frozen in a secret lab underneath the hospital. Tony turned to Luke Spencer for help, and he was able to get Lucas’ cure.
After a few dating mishaps with his adoptive cousins Maxie and Georgie Jones, Lucas came out in 2006. Tony readily accepted Lucas, but died a few months later, which devastated Lucas. He moved to Seattle shortly after, and Bobbie followed.
Bobbie returned to Port Charles when her daughter Carly Jacks went missing. Back in town she learned that Lucas’ mobster bio-dad Julian was still alive. She tried to hide it from Lucas, but Julian had learned about Lucas and was intent on meeting his son. Lucas came back to Port Charles to give Bobbie support and was shocked to learn about Julian. He went out to The Floating Rib for a drink where he met Brad Cooper. The two commiserated over being adopted out of mobster families. The two had a one-night stand. Lucas wanted more, but Brad told him he was interested in another man, Felix DuBois.
When Felix broke a date with Brad, Brad went out for a drink with Lucas instead and they ended up back in bed.
Lucas went to visit Tony Jones’ grave on the anniversary of his death and ran into Julian. Their relationship was already strained, and when Lucas told Julian he was gay, he did not react well. Julian assumed if he had been in Lucas' life Lucas might be straight.

Maxie walks into The Floating Rib and demands to know if Mac or Dante called Immigration on Levi. The men deny doing anything. They debate the merits of Dunkleman. Maxie continues accusing them. Levi and Nathan arrive. Maxie accuses him. Nathan apologizes for dragging them all into this. Levi wants to check his phone. He hands it over and Maxie finds an 800 number. She calls it. It's Immigration. The cop is clueless and Maxie accuses him of being a liar. She declares that he won't get away with this. Meanwhile, Lucas and his father arrive. They say hi to Lulu and Liv, who gives Julian the cold shoulder. Father and son sit down across the room. Julian wants to hear about his problems, even if that includes his love life. He chokes on his beer when Lucas talks about Brad and Felix making out. He explains his romantic conundrum. Before he can explain which man he chose, Felix and Brad walk in together. Lucas is miffed.
Lulu pays Lucas a visit at Carly's. They discuss "The Golden Girls" and he bemoans his man troubles. She warns that triangles always end in disaster. He unpacks the story. He's finished with Brad but he likes all the attention. Lulu urges him not to give up on Brad for her sake if he still wants him. She encourages him to go and talk to the man he wants.
At the hospital, Brad startles Felix while he is having a shower in the locker room. Felix covers up. "You're a hungry, hungry hippo homosexual and you're famished!" Brad accuses. He knows that "The Golden Girls" is foreplay and orders him to back away from Lucas. Felix doesn't appreciate him talking about the "Girls" like that. Brad accuses him of being frigid and then plants a kiss on him. Felix pushes him away. Lucas shows up in time to see this and storms out. Brad chases him down and claims he just got carried away. Lucas wanted to settle things and announce who he was choosing. Felix joins them. Brad bickers with him. Lucas stomps off. Down the hall, Kiki tells Morgan and Tracy that Rafe is brain dead. Tracy bluntly asks if he was an organ donor. He is. Meanwhile, Alice continues trying to tell Michael the truth but Tracy barges in to say she's found an organ ready for the taking. Tracy gloats that Alice owes her for this. She drags Michael off to make enquiries. They ask Kiki about the heart. She offers to talk to Silas about this. Across the hospital, Silas sits at Rafe's bedside vowing revenge if this is Patrick's fault. Nina shows up, claiming she's been worried about Rafe. He fills her in. She tries giving a pep talk but it's hopeless. He stares off. Nina wonders if Dr. Drake is responsible. "Justice will happen," Silas intones. She thinks he should have Sam investigate. He doubts Sam agrees with his suspicions. She can't believe Sam is siding with Drake. Kiki arrives and asks Silas into the hall. She brings up the transplant. Rafe's blood-type matches Alice's. Back in the room, Nina tells Rafe that he's helping her drive a wedge between Silas and his 'trailer trash girlfriend'. She promises he'll get a great funeral. He opens his eyes. She jumps. Elsewhere, Sam corners Patrick at the elevators and asks him if he took revenge on Rafe. They go somewhere quiet and he explains that the voice in his head called for vengeance... but he didn't listen to it. She believes him but doubts convincing Silas will be so easy. Sam says she'll help Silas see things her way. They hug.
