When Rafe Kovich Jr. arrived in Port Charles, he was on the run with his mother, Alison Barrington, a former resident. She had previously fled Port Charles to escape Caleb Morley, whom she thought was the king of the vampires. They spent their lives on the run, and were back in Port Charles looking for help.
Alison was murdered down by the pier shortly after arriving in town, and Rafe was a suspect. He had befriended Molly Lansing-Davis, and she believed in Rafe’s innocence, despite pressure from her boyfriend TJ Ashford not to get involved. Her convictions were strengthened by Lucy Coe, who was a cousin of Rafe’s legal father, Rafe Kovich.
Lucy didn’t believe Rafe was safe in jail while vampires were back in Port Charles, and worked with Molly to help Rafe escape. He hid out in Molly’s bedroom at her mother Alexis Davis’ house.
Meanwhile, Caleb kidnapped Sam Morgan and her son Danny, believing she was his long-lost wife Livvie Locke. John McBain realized that Caleb was really serial killer Stephen Clay who had suffered a mental breakdown after his wife Livvie died. Lucy and John McBain took down Stephen, who was the real person who killed Alison, and Rafe’s name was cleared.
Lucy tried to become Rafe’s legal guardian, but since she was not blood related and was considered unfit after believing in vampires, he was put in a group home. Sam took Rafe in instead, and he helped care for Danny while falling in love with Molly.
Soon, his biological uncle Dr. Silas Clay came to town and wanted custody of Rafe. Sam fought for guardianship of Rafe while Danny was diagnosed with leukemia. When Silas was awarded guardianship instead, he allowed Rafe to stay with Sam until Danny was better. When Danny recovered, Rafe moved in with Silas.
Molly was adamant about just staying friends with Rafe and continued to date TJ. After Rafe and Molly shared a kiss, TJ pressured her to end her friendship with him. They fought, and while they were on a break, TJ took Taylor DuBois to the prom instead of Molly. After seeing what he thought was a romantic moment between Rafe and Molly, TJ slept with Taylor. But Molly and TJ were able to work through their problems much to Rafe and Taylor’s dismay.
Taylor and Rafe decided to pretend to date in order to make Molly and TJ jealous. It worked, but Molly refused to let it alter how she felt about TJ. When Rafe learned that TJ was a witness to a mob shootout at Sonny Corinthos’ coffee warehouse, he told police commissioner Anna Devane despite Molly insisting no one know. After, Rafe worried that his admission put TJ in physical danger.
Molly ended her friendship with Rafe and he began secretly using cocaine that had recently begun flowing into Port Charles via the Jerome crime family.
Sam found out Rafe was the person who ran Dr. Patrick Drake and Sabrina Santiago off the road, causing the death of their infant son Gabriel. Rafe was encourage to run by Nina Clay instead of facing his actions. With Molly in the car, he crashed, but not before telling her someone else made him cause the accident that killed the baby.
After the accident, Rafe needed brain surgery. Patrick performed the operation, during which Rafe became brain dead. After determining his heart was not a viable transplant option for Quartermaine maid Alice Gunderson, he was taken off life support and died July 16, 2014.

At the hospital Tracy and Michael worry how to break it to Alice that she won’t get Rafe’s heart. Michael accuses Tracy of keeping Alice from telling him something, but Tracy doesn’t think now is the time to have a family squabble. He agrees but promises they’ll have this conversation soon. Tracy enters Alice’s room as Felix leaves and tells her there’s been a complication. She promises to find Alice a better heart but wants her to keep quiet about Tracy’s return to ELQ. Meanwhile. Liz takes care of Rafe in his room and despairs about his wasted life, like Jake. Sam enters and Liz offers to have another doctor turn off the machine so Silas doesn’t have to. Sam admits she doesn’t know what Silas needs any more. Liz heard about Nina moving in and Sam tells her that Silas accused Patrick of intentionally leaving Rafe brain dead. Sam wants to convince Silas that Patrick didn’t do it on purpose. Meanwhile, Silas tells Nina he heard her waking up with night terrors, shouting out Rafe’s name. She doesn’t want Silas to worry about her and promises they will get through it together. He wheels her to PT and leaves. Rosalie approaches and makes Nina jump. Nina admits if it weren’t for her Rafe would still be alive. Sam, Kiki – they all did her wrong. But not Rafe. Nina wants to confess everything to Silas. Rosalie accuses Nina of still being in love with Silas, so if she tells him now she will lose it all. Later, Silas enters Rafe’s room and Sam tells him his heart can’t be used for the transplant because of the heroin. Later, Liz and Felix catch up in the locker room. Felix is still shocked Britt put in a good word for her, even if it was to look good for Nik. He wonders how Liz would feel about Britt and Nik getting back together.
Back at the hospital, Molly remembers someone put Rafe up to driving the car off the road when she was sleeping but forgets when she wakes up. She wants to ask Rafe what he said and Alexis and TJ tell her that Rafe is brain dead. Molly is overwrought and screams he didn’t deserve this. They tell her he’s being taken off life support today and Molly wants to be there to say goodbye. Meanwhile, Michael returns to Alice’s room and overhears Tracy ask Alice if they have a deal. Alice tells Michael they just agreed to both do whatever it takes to find a new heart. Silas, Sam, Molly, Alexis, TJ and Kiki gather in Rafe’s room. Silas thanks Molly for caring about Rafe and they all take turns saying their goodbyes. Molly tucks a book under Rafe’s arm and apologizes for everything that went wrong. She hopes he knows how much he meant to her. Silas turns off the machine with tears streaming down his face. Everyone holds each other and waits until Rafe flatlines. Back in her room Molly thinks remembering what Rafe said will give her closure. Meanwhile, Rosalie wheels Nina to find Silas and she sees him holding Sam.
At the hospital, Dr. Obrecht corners Franco and tells him how great he's been for the pediatrics ward. He's happy to be contributing to society. He goes back to work. Meanwhile, Tracy is anxious for news about Rafe's heart. She tells Monica about it. Everyone tries to keep her calm. Down the hall, Kiki and Silas discuss the transplant. Rafe's blood-type is a match for Alice. She apologizes for the timing of this. He doesn't see what difference the timing would make and agrees to talk to Alice's people. When they find them, Silas says he's willing to donate his nephew's heart. Franco strolls into the middle of the conversation. They tell him to get lost and he comments on how rude the Quartermaines are. After he leaves, Silas says that Rafe had a good heart, in spite of any mistakes he made. Monica is optimistic that things will work out. Morgan is thrilled and gives Kiki a hug. Michael stares at them. When Monica and Silas start comparing Rafe and Alice's heart in his office, Obrecht interrupts and wonders what they are up to. They fill her in. "No death wasted," Obrecht declares as her motto. The doctors return to the family and tell them that Obrecht has signed off on the transplant. A moment later, Obrecht interrupts to declare there is a problem. She explains that Rafe tested positive to heroin and the hospital can't allow a transplant in that situation. In Rafe's hospital room, Nina thanks Rafe for helping her drive a wedge between Sam and Silas. His hand moves and he stares at her. She keeps herself calm, but Rafe revives and yanks the oxygen tube out of his mouth. He taunts her and threatens to expose her secrets. She yells threats until Franco interrupts. She snaps out of it. Rafe is still comatose. Franco tells her that he used to see and hear things too. He hopes she won't be destroying lives like he did but he assumes she's just under a lot of stress. He leaves as Silas arrives.
