At the church, a befuddled Anna searches for Marlena but finds John. She tells him Stefano's awake. John has catching up to do with him. Anna worries but John asks her not to. "He won't hurt you Anna, you have my word." He asks about Marlena and Anna says she hasn't returned any of her phone calls. He thinks he knows where she is and makes a beeline for the door. Anna follows.

In Stefano's room, Tony doesn't believe that Marlena could have harmed Stefano. "This woman, this she-devil is nothing but a well constructed façade," Stefano complains. EJ tells Stefano they're just trying to comprehend so they can help. They can't, Stefano says. He rages that he was trapped like a rat. He couldn't move or talk. He's going to repay Marlena in kind. Sami arrives and can't believe Stefano's really awake. She wants to know what he did with her mother. Stefano is not happy to see that she and EJ are no longer married. Sami yells that she won't let him get away with whatever he is planning.

EJ takes Sami out of the room and tells her there is nothing to be gained from this. Sami says she has called Marlena over and over and she is gone. "Stefano has done something to her." They argue that Stefano's minions did something and EJ lies to Sami. Just because Stefano's awake and Marlena can't be found, doesn't mean Stefano went after her! Sami can tell that EJ has a bad feeling, but he denies it and offers to help find Marlena.

Inside Stefano's room, Tony yells at Stefano for harming Marlena, an innocent woman. He demands to know what Stefano did, but Stefano yells at him for interrogating him. He feels insulted but Tony's not deterred. He continues his rant. Stefano asks where his loyalty is. It's to himself and Anna, points out Tony in anger. Stefano tells Tony that Marlena tortured him, injected him with drugs that induced catatonia. "I was trapped in my own body." He's going to make Marlena pay for the rest of her life, he growls. Tony doesn't think Marlena is capable. Is there proof? Stefano is livid. "My word is not sufficient?!" Tony doesn't know what to think. With tears in his eyes, Stefano explains that he heard everything that was said around him and it was hell. Tony wonders if he had an adverse reaction to the truth serum, but Stefano, through a fit of coughing, denies this. Tony thinks Stefano should feel blessed that he's awake and asks him to think about his legacy. "That you are the bigger man." He wants him to take the high road for once, but Stefano claims that's too late.

Marlena lies on the floor of her townhome, motionless. "Help…me," she whimpers.

John and Anna arrive at the mansion, but can't find Marlena, or Rolf. Anna recounts how she went to the townhouse. Marlena didn't answer the door and her neighbors haven't seen her. John decides to break in, and they start out for the townhouse, but John's tires are cut on his car. Rolf must have done it. Anna's shocked, but John knows that Rolf would do anything for Stefano. Anna calls him insane. "He's as sane as I am, but he's a coward," replies John. EJ has his other car, but John has a bike. He gives her a helmet and tells her she's coming. As she teeters out the door, Anna points out she's wearing heels and asks if they can't just call a cab.

Later, John and Anna arrive at the townhouse and spot a syringe beside an unconscious Marlena, sprawled on the floor. Marlena's not breathing so John gives her CPR. Anna panics but calls 911, just as Sami and EJ show up. Sami rushes to John's side. "Mom? What happened?" Sami cries hysterically and tells her mother to fight. "We need you," Sami says. Marlena opens her eyes and takes a breath and John stops CPR.

Inside Stefano's room, Tony spots Anna, outside the room, crying. He rushes to her and she tells him what happened. She thinks this is Stefano's doing. She's angry with Tony for keeping quiet about Stefano being awake. Her hands go to her hair and she asks if she has helmet hair. Tony says she looks great and admires her nerve and kindness. Anna tells him she is proud of him and his strength, they kiss, and Tony says he has to see Stefano.

Marlena is taken to a room in ER. Kayla examines her and they discuss who did this. Kayla wants John to stop this. He heads to see his brother but Sami tells him Marlena needs him right now.

Inside Marlena's room, a nurse tells Kayla that they suspect poisoning. They found a syringe next to her. Kayla tells Marlena to fight with everything she has and tells John they'll run tests. "You people are good at that," he comments, snidely. Kayla yells that the old John would have done all he could to protect Marlena. She admits she saw him at the church and he stood around and did nothing while Bo was taken away. It was almost as if he reveled in it! John tells her she saw what she wanted to see. He tells her to focus on her patients, but she tells him she knows he is a good man and has seen glimmers of this with the heroism at the plane crash and with saving Marlena's life. She thinks his feelings for Marlena are in there somewhere. "She is and always will be the great love of your life." John stares off for a time and then goes to Marlena.

John stands at Marlena's side as Sami watches from outside. She goes to him and tells John that her mom's usually the one taking care of everyone else. "Yes," says John, uncomfortably. Sami tells John she thinks he loves Marlena, whether he knows or not. She apologizes, knowing it must be frustrating to hear this.

Tony goes to Stefano. "Why'd you do it?" Stefano asks if she's alright. Tony is sickened that it brings Stefano so much satisfaction to hurt others. Tony thinks if Marlena did hurt him, she did so to protect her family. She's now fighting for her life, he yells. Stefano reminds Tony that he's been tortured for months. Tony wonders why he bothers and Stefano tells him to get out of his sight. The two start yelling at each other and Tony yells that Stefano's about power and revenge. Stefano thinks Tony's a fool and this is about family. "Our family was attacked," he says and Tony had better decide whose side he's on. The lines are about to be drawn. Stefano has more in store.

Anna goes to Kayla. She's praying for Marlena and asks if Bo and Hope are alright. Kayla doesn't know. She and Steve didn't get a chance to talk to them after, but she's not optimistic. She knows Bo puts family first. She thanks Anna for saving Marlena, but Anna thinks John's the real hero. Kayla thinks they're both responsible.

Anna and EJ sit. She notices he's looking tired. He's exhausted and hasn't been sleeping. Sami goes to them and asks if she can talk with EJ alone. After they leave, Anna prays to God to protect them from Stefano.

Next on Days Of Our Lives:

Steve asks, "What happened to Marlena?" Kayla says, "She's been injected with a paralyzing toxin." Marlena quietly mumbles, "Stefano!"

Sami turns to EJ, "As soon as they find out what Stefano did to my mother I hope John kills him!"

John says, "So it is to be." Stefano says, "Hello, John."
