Jake learns surprising information about Cara, David shows signs of life and Kendall lays it out for Ryan.
Griffin meets with Angie at ConFusion and is offered a more permanent role at the hospital. He tells her it's not his thing and when his time is done at the Miranda Center he will move on. She tells him saving the world is admirable, but saving it one person at a time is too. Before Angie leaves, Griffin tells her he'd like to scrub in for some surgeries. Griffin then sees Tad sitting at the bar and tells him he just turned down a full time position at the hospital. Tad questions him about his past and where he's from. He also wonders if when he leaves again, will he be taking Cara with him? Griffin assures him Cara doesn’t stay in one place too long. Once Griffin is gone, Tad looks up information on Griffin's hometown. He says, "It can't be right. Or can it?"
Cara finds Jake at the hospital to again ask him back to Doctor's Without Borders. He tells her to stop with her pitch. She assumes what they had together isn't even on his radar anymore because of how happy he is now. He responds that's not true. She gets back to trying to recruit him and Jake admits he was lost without Doctors Without Borders when he first came back, but he found his passion in Amanda and Trevor. She's happy for him, which is all she ever wanted. Jake goes off on her about leaving him for Griffin, but Tad shows up and tells Jake Griffin isn't Cara's lover, he's her brother. Tad tells him he did some digging after finding a photo of two kids in Griffin's wallet and that Cara changed her name. Cara explains she was wearing a Dr. Finn's badge when he met her. Her family wanted her in a safe doctor's office, but she wanted something more and he showed it to her. Jake wonders if there was anything she told him that was true. She tells him she was too young to get married so she called her brother to come get her. She wanted to experience new things and new people and she a clean break. He can't believe she made him think she was in love with someone else so he wouldn't come after her. She calls coming here a big mistake but says she's sorry before she leaves.
Jake finds Tad and fills him in on what Cara said. Jake doesn't know how to process what he just learned. Tad thinks all that matters is the end result and what he has now. Jake thanks his brother for looking out for him. Tad says always. Jake then sees Amanda and says, "Thank God for you," and kisses her.
Griffin finds a drunk Cara at ConFusion who tells him Jake knows they are related. She has to get out of there and he wishes he could go with her. She doesn’t need to be protected anymore and doesn't want to be treated like a glass doll. She gets a call and learns there's an immigration issue with DWB so she can't leave tonight.
Elsewhere at ConFusion, Madison expresses her fears about having a baby to Randi and Frankie. She knows the clock is ticking and that she has to make this decision on her own.
At Krystal's with Kendall, Ryan and the boys, Greenlee gets called to the hospital for issues with David. Greenlee leaves and Ryan asks Kendall to talk. He wants to help her decorate for Christmas and she agrees, but tells him not to bring Greenlee. Ryan thinks she needs Greenlee. Kendall knows it's wrong to exclude her, but every time she seems them together it reminds her of what she won't ever have again with Zach. Ryan reminds her he was there when Zach's plane went down too and wonders if she feels the same about him. If it weren't for Spike, she wouldn't want anything to do with him either. Ryan wants to be respectful of what she's going through, but they want to be part of her life. She wants to talk about the holidays later and tells Spike to say goodbye to his Daddy.
Greenlee meets with Frankie at the hospital and learns David's monitors are registering different readings. She assumes it means he could be waking up. Frankie can't be sure, but tells her she was notified because they have to tell the next of kin when there are any changes and then he gets paged away. Greenlee tells a comatose David Kendall hates her because of him so she will never be free of him. She sees his finger move and she calls for Frankie. Frankie returns to check David out and says medically there's been no change. Frankie leaves and Greenlee urges her husband to be a better person if he wakes up from this. She walks out and David's hand moves again.
Greenlee and Ryan meet up at home and she tells him David's finger moved. Ryan in turn tells her Kendall doesn't want her at Christmas.
